
PCOS and Breast Cancer: What the Research Says
If you have PCOS, or its newly renamed form, PMOS, and you’ve found yourself down a late-night search spiral about breast cancer risk, you’re not alone. It’s one of the most commonly asked questions in gynaecology consultations. Both conditions involve hormones. Both affect women. So the question makes sense.
The honest answer is: the research is mixed, and that’s worth understanding clearly.
What PMOS Is and Why Hormones Matter Here
In PMOS, three hormonal disruptions are particularly relevant when thinking about breast cancer risk:
- Elevated oestrogen exposure: With irregular ovulation, there’s no progesterone to balance oestrogen. This results in longer periods of unopposed oestrogen exposure, and breast tissue is sensitive to oestrogen.
- Elevated androgens: Higher androgen levels, a defining feature of PMOS, can be converted to oestrogen in body fat. The more body fat, the more conversion. This further raises oestrogen availability in breast tissue.
- Insulin resistance: High insulin levels stimulate the ovaries to produce more androgens, and independently, high insulin is associated with increased cell proliferation in breast tissue
None of this means PMOS directly causes breast cancer. But the hormonal environment in PMOS creates conditions that may, over time and alongside other factors, influence breast cancer risk.
What the Research Actually Shows
Mendelian randomisation in Breast Cancer Research and Treatment found PCOS to be associated with a 9% increased risk of oestrogen receptor-positive (ER-positive) breast cancer. In relation to ER-negative breast cancer, no correlation was detected. This is relevant since ER-positive breast cancer is dependent on oestrogen signaling and therefore fits with the hormonal theory mentioned above.
What Really Causes Breast Cancer in Women with PMOS
- Obesity is seen frequently in PMOS because of insulin resistance, which causes increased production of oestrogen and is known to be a risk factor for breast cancer, especially post-menopause.
- Type 2 diabetes and insulin resistance: Insulin, by itself, causes cell growth in the breast. Women with PMOS are at significantly increased risk for both.
- Irregular ovulation and oestrogen exposure: Less ovulation causes prolonged exposure to oestrogen, the same process by which women with PMOS have an increased risk of endometrial cancer
Myths Associated With PMOS and Breast Cancer Risk
Myth 1: “PMOS puts me at certain risk of getting breast cancer.”
Fact: The studies do not show this association. PMOS has not been categorized as one of the high-risk conditions for breast cancer by any existing clinical guideline.
Myth 2: “Since PMOS is associated with elevated levels of oestrogen, I have the same risk of developing breast cancer as women on hormone replacement therapy.”
Fact: Oestrogens that circulate in the body of patients with PMOS and oestrogens used in hormone replacement therapy act similarly but not identically.
Myth 3: “By treating my PMOS, I eliminate my risks of breast cancer.”
Fact: Although treatment of PMOS, including lifestyle modifications, can help normalize hormone-related mechanisms of breast cancer development, there are other risk factors, and they cannot be eliminated.
Myth 4: “I am a young woman with PMOS, so breast cancer risk is not an issue for me.”
Fact: Even though breast cancer becomes more frequent among older women, it depends on the hormonal milieu created during many years of PMOS existence. Building healthy habits like regular screening and maintaining proper weight will prove valuable in the future.
The link between PMOS and breast cancer is real in biological terms, plausible in mechanism, and still being quantified in its clinical significance. But managing PMOS well protects reproductive and metabolic health.
